Semont Maneuver

Alternative Names: Libertary manuever

The Semont maneuver (also called the liberatory maneuver) is another treatment for BPPV, especially for patients who can’t tolerate the slower movements of the Epley maneuver. It’s a quick series of head and body movements designed to dislodge the crystals from the semicircular canals of the inner ear.

Here’s how the Semont maneuver generally works (for right-sided BPPV as an example):

-Sit upright on an exam table with legs dangling.

-Turn the head 45 degrees to the left (away from the affected right ear).

-Quickly fall onto the right side (affected side) so that you’re lying on your side with your nose pointing upward.

-Stay in that position for about 30 seconds to 1 minute (or until dizziness stops).

-Then, in one quick motion, move to lying on the left side (opposite side) without changing head position, so the nose now points downward.

-Stay there for another 30 seconds to 1 minute.

-Slowly return to sitting up.

The Semont maneuver is reported to be an effective treatment for posterior canal BPPV, working by rapidly moving the patient between positions to reposition displaced inner ear crystals. Based on my review of the medical literature, it can provide meaningful symptom relief when performed correctly. However, it is not commonly performed in my emergency department.
