Entacapone (Comtan®) is a COMT inhibitor used in Parkinson’s disease to enhance the effects of levodopa/carbidopa. By blocking levodopa breakdown in the body, it helps more medication reach the brain and prolongs “on” time. It does not work alone, but when added to levodopa it can reduce “wearing-off” fluctuations and improve daily symptom control. Side effects may include diarrhea, nausea, orange-colored urine, and increased dyskinesias from higher dopamine levels. Entacapone is most useful in patients with advanced disease who struggle with motor fluctuations.
