Official Rules and Terms & Conditions

NO PURCHASE NECESSARY. A PURCHASE WILL NOT IMPROVE A PARTICIPANT’S RANKING OR OPPORTUNITY TO RECEIVE A PRIZE.

1. Sponsor

The Curbside Founding Voices Contest (the “Contest”) is sponsored and administered by Curbside Tech Inc., operator of Curbside.bio (the “Sponsor”).

2. Contest Period

The Contest begins on August 4, 2026, at 12:00 a.m. Pacific Time and ends on August 31, 2026, at 11:59 p.m. Pacific Time (the “Contest Period”).

Only qualifying activity submitted to and successfully recorded by Curbside.bio during the Contest Period will count. The Sponsor’s systems and records will be the official timekeeping method.

3. Eligibility

The Contest is open only to individuals who, at the time of participation:

  • Are legal residents of one of the 50 United States or Washington, D.C.;
  • Are at least 18 years old;
  • Provide a valid email address associated with their qualifying Curbside.bio activity; and
  • Comply with these Official Rules and applicable Curbside.bio policies.

Employees, officers, directors, and contractors of the Sponsor; persons directly involved in administering or reviewing the Contest; and their immediate family or household members are not eligible to receive a prize.

The Contest is void outside the 50 United States and Washington, D.C., and wherever prohibited or restricted by law.

4. No Purchase Required

No purchase, payment, paid subscription, or fee is required to participate. A purchase will not provide additional points or otherwise improve a participant’s ranking. Internet access and a compatible device may be required, and participants are responsible for any third-party data or internet charges.

5. How to Participate

During the Contest Period, eligible individuals may participate by visiting Curbside.bio and completing qualifying activities through the platform. Qualifying activities may include:

  • Submitting a star rating;
  • Submitting a written treatment review;
  • Commenting on another review;
  • Receiving a qualifying helpful vote on a submitted review;
  • Receiving a qualifying comment on a submitted review; and
  • Completing qualifying activity on an active participation day.

Participation and scoring occur exclusively on Curbside.bio. Social-media likes, comments, follows, shares, reposts, tags, or messages do not count toward eligibility, points, rankings, or winner selection.

6. Email Requirement and Verification

Qualifying activity must be associated with a valid email address that allows the Sponsor to identify the participant, combine the participant’s qualifying activity, and contact the participant.

An email address does not need to be verified when a contribution is first submitted. However, before receiving a prize, a potential winner must demonstrate control of the email address associated with the qualifying activity. The Sponsor may verify control by requiring the potential winner to reply to the winner-notification email, log in to the associated account, or complete another reasonable verification step.

Activity associated with an invalid, inaccessible, duplicated, or unverifiable email address may be excluded if the Sponsor cannot reliably identify or contact the participant.

7. Genuine Experiences and Content Requirements

Ratings, reviews, and comments must be truthful, genuine, and based on the participant’s own treatment experience, genuine caregiving experience, or appropriate professional experience. Healthcare professionals must not disclose confidential or identifying patient information.

Participants may submit multiple genuine reviews through the ordinary functionality of Curbside.bio. Each submission must independently satisfy these Official Rules. Duplicate, substantially repetitive, fabricated, copied, misleading, irrelevant, or otherwise ineligible content may be removed and will not earn points.

Reviews reflect individual experiences and opinions. They are not medical advice and are not endorsements or recommendations by the Sponsor.

8. Protection of Personal and Health Information

Participants must not disclose another person’s confidential, protected, or identifying information, including names, dates of birth, medical-record numbers, contact information, street addresses, identifying photographs, or other information that could reasonably identify another patient or individual.

Participants should submit only information they are comfortable sharing through Curbside.bio.

9. Points System

Eligible participants will receive points for qualifying activity completed during the Contest Period as follows:

9.1 Review points are not cumulative

Each individual review receives only the highest applicable review score. A star-only rating receives 2 points total; a rating with a written review of 1–50 characters receives 3 points total; and a rating with a written review of more than 50 characters receives 5 points total. A detailed written review does not receive 2 points for the star rating plus 3 points for written content plus 5 points for being detailed.

9.2 Character count and quality

Spaces and punctuation may be included in the character count. Content that is repetitive, meaningless, irrelevant, copied, fabricated, or added primarily to meet a character threshold may be rejected or scored at a lower level. The Sponsor may consider substance and relevance, not character count alone.

9.3 Comments

A qualifying comment must relate meaningfully to the review or treatment being discussed, reflect genuine engagement, and comply with Curbside.bio’s policies. Repetitive, generic, copied, automated, promotional, harassing, or irrelevant comments will not qualify.

9.4 Helpful votes and comments received

Helpful votes and comments received are not subject to a fixed points cap at this time. They will count only when the Sponsor reasonably determines that they reflect genuine activity by another user.

Engagement may be excluded if it appears coordinated, exchanged, automated, purchased, duplicated, generated through related accounts, or created primarily to manipulate Contest rankings. A participant may not earn points from commenting on the participant’s own review.

9.5 Active participation day

An “active participation day” is a calendar day during which a participant completes at least one qualifying activity on Curbside.bio. A participant may earn no more than one active-participation point per calendar day, regardless of the number of qualifying activities completed that day. Calendar days are determined using Pacific Time.

9.6 Removed or deleted activity

Points may be removed if the underlying contribution is deleted, removed, found to violate these Official Rules, determined to be duplicative or inauthentic, or affected by a technical or scoring error.

10. Leaderboard

Curbside.bio may display preliminary rankings during the Contest Period. Leaderboard totals are unofficial until the Sponsor completes its review and verification after the Contest Period ends.

The Sponsor may correct scoring or technical errors, remove duplicate or ineligible points, exclude invalid contributions, adjust totals associated with deleted or disqualified activity, investigate suspicious activity, and delay or suspend leaderboard updates during review. Appearing on the leaderboard does not guarantee eligibility or receipt of a prize.

11. Winner Determination

After the Contest Period ends, the Sponsor will review the activity of the highest-ranking participants. Subject to eligibility verification and compliance with these Official Rules:

  • The eligible participant with the highest verified point total will be the potential first-place winner;
  • The eligible participant with the second-highest verified point total will be the potential second-place winner; and
  • The eligible participant with the third-highest verified point total will be the potential third-place winner.

Winners are determined by verified points earned through qualifying activity and are not selected randomly.

12. Ties

If two or more eligible participants have the same verified point total for a prize position, the Sponsor may, in its sole discretion and subject to applicable law, elect to award an additional prize of the same type and value to each tied participant. The Sponsor is not obligated to award duplicate prizes.

If the Sponsor does not elect to award duplicate prizes, the tie will be resolved in the following order:

  • The participant with the greatest number of qualifying detailed written reviews containing more than 50 characters;
  • If still tied, the participant with the greatest total number of qualifying written reviews;
  • If still tied, the participant who reviewed the greatest number of distinct treatments; and
  • If still tied, the participant who reached the tied final point total first, according to the Sponsor’s records.

No random drawing will be used to resolve a tie. If the Sponsor awards duplicate prizes, doing so will not alter the placement or prize entitlement of any participant who ranked above the tied position.

13. Prizes

The total approximate retail value of the three stated prizes is $175. If the Sponsor elects to award duplicate prizes because of a tie, the total prize value may increase.

Prizes will be delivered electronically, are not transferable, may not be assigned to another person, and may not be redeemed for cash through the Sponsor. The Sponsor may substitute a prize of equal or greater value if a stated prize becomes unavailable.

Amazon.com, Inc. is not a sponsor or administrator of the Contest. Amazon and related marks are trademarks of Amazon.com, Inc. or its affiliates.

14. Potential-Winner Notification and Verification

Potential winners will be contacted at the email address associated with their qualifying Curbside.bio activity within approximately 10 business days after the Contest Period ends. A potential winner must respond within seven calendar days after the initial notification.

Before receiving a prize, a potential winner may be required to confirm or provide:

  • The potential winner’s legal name;
  • Confirmation that the potential winner is at least 18 years old;
  • Confirmation that the potential winner is a legal resident of one of the 50 United States or Washington, D.C.;
  • The preferred email address for electronic prize delivery;
  • Control of the email address associated with the qualifying activity;
  • Compliance with these Official Rules; and
  • Other limited information reasonably necessary to verify eligibility, maintain legally required records, or comply with applicable law.

A mailing address is not required to deliver an electronic gift card. The Sponsor may request address information only if reasonably necessary for eligibility verification or legal compliance.

A potential winner may be disqualified if the person cannot be contacted, does not respond on time, does not complete reasonably requested verification, is found ineligible, violated these Official Rules, or declines the prize. The Sponsor may then offer the prize to the next-highest eligible participant.

15. Electronic Prize Delivery

Amazon eGift cards will be delivered to the email address confirmed by each verified winner. The Sponsor is not responsible for an incorrect email address supplied by a winner, email filtering or delivery problems outside the Sponsor’s control, unauthorized access to a winner’s email account, or loss or misuse of a gift-card code after successful delivery.

A winner should contact info@curbside.bio promptly if the prize email is not received.

16. Taxes

Winners are responsible for any federal, state, or local tax obligations associated with accepting a prize. The Sponsor may request tax information only when required by applicable law.

17. Prohibited Conduct and Disqualification

The Sponsor may reject contributions, remove points, suspend an account, or disqualify a participant for conduct including:

  • Submitting false, fabricated, misleading, copied, plagiarized, duplicate, or substantially repetitive content;
  • Creating or using multiple accounts or identities to manipulate activity;
  • Using bots, scripts, automation, or other methods to generate inauthentic activity;
  • Using artificial intelligence to fabricate a personal treatment experience;
  • Submitting meaningless or irrelevant content solely to earn points;
  • Artificially generating or coordinating helpful votes, comments, or other point-generating activity;
  • Offering compensation or another benefit in exchange for engagement;
  • Impersonating another person;
  • Disclosing another person’s private, confidential, or identifying information;
  • Harassing another participant;
  • Attempting to manipulate the leaderboard;
  • Interfering with the operation or security of Curbside.bio;
  • Violating these Official Rules or applicable Curbside.bio policies; or
  • Engaging in conduct that undermines the fairness, integrity, or intended purpose of the Contest.

The Sponsor may consider the content, timing, frequency, technical information, and overall pattern of activity when evaluating authenticity. The Sponsor’s reasonable, good-faith decisions concerning eligibility, scoring, authenticity, and compliance will be final, subject to applicable law.

18. Social-Media Activity Does Not Count

The Contest is conducted exclusively through qualifying activity on Curbside.bio. Participants are not required to like, comment, follow, share, repost, tag anyone, or send a social-media message to participate.

Social-media posts are used only to communicate information about the Contest and direct interested individuals to Curbside.bio.

19. Social-Media and Third-Party Disclaimer

The Contest is sponsored and administered solely by Curbside Tech Inc. It is not sponsored, endorsed, administered by, or associated with LinkedIn, Facebook, Instagram, Meta Platforms, Inc., Amazon.com, Inc., or any other social-media or third-party platform on which it may be mentioned.

Questions or complaints concerning the Contest must be directed to info@curbside.bio and not to any social-media platform or gift-card issuer.

To the extent permitted by law, each participant releases Facebook and Instagram from claims arising from the administration of the Contest.

20. Privacy

Curbside.bio may process limited information associated with Contest participation, including an email address, account or participation identifiers, display name, submitted ratings, reviews and comments, dates and times of qualifying activity, information used to calculate points, and technical or account information reasonably used to detect fraud or manipulation.

Additional information may be collected privately from potential winners solely to verify eligibility, communicate with winners, deliver prizes, prevent fraud, maintain legally required records, and comply with applicable law.

Winner-verification information will not be displayed publicly or used for unrelated marketing without permission. Information will be retained only as long as reasonably necessary or legally required. Prize communications will be sent from info@curbside.bio.

Publicly submitted reviews and comments may remain available on Curbside.bio after the Contest ends, subject to Curbside.bio’s applicable Terms of Use, Privacy Policy, moderation practices, and legally required rights.

21. Ownership and Use of Contributions

Submitting a contribution does not transfer ownership of a participant’s original content to the Sponsor. By submitting content through Curbside.bio, the participant grants the Sponsor the permissions described in Curbside.bio’s applicable Terms of Use.

Participation in the Contest does not grant the Sponsor additional rights to use a participant’s legal name, likeness, or treatment story in advertising unless the participant separately agrees.

22. Winner Publicity

The Sponsor will not publish a winner’s legal name, mailing address, private email address, or other private information without permission, except when disclosure is required by law.

The Sponsor may announce a winner using the winner’s existing public Curbside.bio display name, provided that the display name was already selected for public display. A winner may request that no public announcement identify the winner by display name.

23. Technical Problems

The Sponsor is not responsible for interrupted or unavailable internet access, website outages or delays, failed or incomplete submissions, incorrectly recorded activity, unauthorized interference, email delivery failures, device or browser incompatibility, or other technical circumstances outside the Sponsor’s reasonable control.

If a technical problem materially affects scoring, the Sponsor may correct affected totals using reasonably available records.

24. Suspension, Modification, or Cancellation

The Sponsor may suspend, modify, or cancel the Contest if fraud, technical failure, unauthorized interference, security concerns, legal requirements, or circumstances beyond the Sponsor’s reasonable control materially affect its fairness, integrity, or operation.

The Sponsor will not materially change the scoring or winner-selection criteria after the Contest begins merely to favor or disadvantage a participant. Any material change will be communicated through a reasonable public notice.

If the Contest ends early, the Sponsor may determine winners based on eligible, verified points earned before suspension or termination, when reasonably fair and legally permitted.

25. Release and Limitation of Liability

To the fullest extent permitted by applicable law, participants agree that the Sponsor and its officers, directors, employees, contractors, and agents will not be responsible for losses or claims arising from technical failures, delayed or incorrectly recorded activity, unauthorized interference, incorrect information supplied by a participant, a participant’s violation of these Official Rules, acceptance or use of a prize, or the conduct of another participant or third party.

Nothing in these Official Rules excludes or limits liability that cannot lawfully be excluded or limited.

26. Governing Law and Disputes

These Official Rules are governed by the laws of the State of California, without regard to conflict-of-law principles. Any dispute will be handled under the applicable dispute-resolution provisions in the Curbside.bio Terms of Use, to the extent those provisions legally apply.

27. Winners Information

After the Contest is completed and winners are verified, a person may request the winners’ public display names and the prizes awarded by emailing info@curbside.bio. No home address, private email address, legal name, or other private winner information will be publicly disclosed except as required by law.

28. Acceptance of Rules

By participating, each participant confirms that the participant has read and understood these Official Rules, meets the eligibility requirements, agrees to comply with these Official Rules, and agrees to comply with applicable Curbside.bio terms and policies.

29. Severability

If any provision of these Official Rules is determined to be invalid or unenforceable, the remaining provisions will remain in effect to the fullest extent permitted by law.
